Upcoming Medi-Cal Changes

The Work and Community Engagement Rule

Starting January 1, 2027

  • Some adults ages 19–64 will need to work, volunteer, go to school, attend a job-training program, or have an income to keep Medi-Cal.
  • You can be excused from this rule if you:
    • Have a serious, complex, or ongoing medical condition that keeps you from meeting this rule.
    • Are a caregiver for someone with a disability or serious medical or mental health condition.
    • Have a serious mental health condition that keeps you from meeting this rule.
    • Have a disability that keeps you from meeting this rule.
    • Are pregnant or were pregnant in the last 12 months, or qualify for postpartum care.
    • Overuse or misuse of drugs or alcohol, are in treatment, or have been asked to attend treatment.
    • Are a parent, guardian, or caretaker of a child who is 13 or younger.
    • Are a veteran with a full (total) disability.
    • Were in foster care on your 18th birthday and are younger than 26.
    • Are receiving CalFresh and are not excused from the CalFresh work program.
    • Are American Indian or Alaska Native.
    • Are in jail or prison.
    • Are meeting the CalWORKs work requirement.

The 6-Month Renewal Rule

Starting January 1, 2027

  • Some adults ages 19–64 will get their Medi-Cal eligibility renewed every 6 months to make sure they still qualify for it.
  • You will be excused from this rule if you:

    • Are American Indian or Alaska Native.

    • Are pregnant or were pregnant in the last 12 months, or qualify for postpartum care.

    • Were in foster care on your 18th birthday and are younger than 26.

How to stay covered?

The most important step is simple:

respond on time.

A lot of people lose coverage because a renewal notice goes to an old address, a form is missed, or paperwork is turned in late. Keep L.A. Covered helps you avoid that.

Watch for Medi-Cal Notices

  • Always read notices from Medi-Cal and respond quickly. Notices may arrive by mail, phone, or text.

  • Keep an eye out for yellow or white envelopes.

Know Your Renewal Date

  • You may be asked to renew your Medi-Cal every six months.

  • Respond before the deadline to help prevent a gap in your coverage.

Keep Your Records

Keep records if you may need to report work, school, training, or volunteer hours.

If You Miss Your Renewal Deadline, Your Coverage Can Stop

In some cases, you may have 90 days to turn in what is needed and restore your coverage without reapplying from scratch. However, it is much easier to avoid missing the deadline in the first place.
